Petitions
Petitions are submitted when you want to request a waiver or exception from a program, college, College of Graduate Studies or University requirement. All petitions should be submitted to the Advising Office (JH 170; essc@csuohio.edu).
ESSC Petition (Education program requests)
Levin College Petition (undergraduate student requests for an incomplete extension, late withdraw, or retroactive registration)
Levin College Readmission Petition (undergraduate and graduate request for readmission after the specified separation time)
Course Relevancy Form (required to extend the 6 or 10-year limit on graduate coursework)
